Program Application Process -- The following Documents, as applicable, need to be downloaded, named, completed and uploaded in HTFOnline as one Compressed ZIP folder (i.e. AGENCY_KEY_ESGApp.zip).
You must use Adobe Reader to complete these documents! Before you begin, make sure to update your ADOBE READER software to the latest version (Adobe Reader X). Otherwise, the form fill, save and sign features will not work properly!
- ESG List of Documents DOWNLOAD (1 PDF Document) - This form is optional and may be of use to applicants for the purpose of tracking documents to be included in the zip folder.
- 2012 Application Summary and Checklist DOWNLOAD (1 PDF Document) - Digital signature required. Do not scan this document. Use your agency's HMIS Agency Key and name as follows: AGENCY_KEY_Summary.pdf
- Local Approval Form(s) DOWNLOAD (1 or More PDF Documents, as needed) - Scan signed form(s) into one PDF document. Use your agency's HMIS Agency Key and name as follows: AGENCY_KEY_Approval.pdf
- Consolidated Plan Consistency Form(s) DOWNLOAD (1 or More PDF Documents, as needed) - Scan signed form(s) into one PDF document. Use your agency's HMIS Agency Key and name as follows: AGENCY_KEY_Consistency.pdf
- Application I, Emergency Shelter DOWNLOAD (1 or More PDF Documents, as needed) - Do not scan this document. Use your agency's HMIS Program Key and name document or documents as follows: PROGRAM_KEY_Shelter.pdf
- Application II, Transitional Housing DOWNLOAD (1 or More PDF Documents, as needed) - Do not scan this document. Use your agency's HMIS Program Key and name document or documents as follows: PROGRAM_KEY_Transitional.pdf
- Application III, Supportive Services Only DOWNLOAD (1 PDF Document, as needed) - Do not scan this document. Use your agency's HMIS Program Key and name document as follows: PROGRAM_KEY_Services.pdf
- Application IV, Homelessness Prevention DOWNLOAD (1 PDF Document, as needed) - Do not scan this document. Use your agency's HMIS Program Key and name document as follows: PROGRAM_KEY_Prevention.pdf.
- Note that there is no funding limit for the Homelessness Prevention program. DCA expects applicants to develop a reasonable business plan for their program based on match resources, the projected number of households to be served, costs per household and the cost of service personnel necessary to effectively carry out the program. In making awards, DCA reserves the right to scale down proposals.
- In designing Homelessness Prevention programs, applicants are advised to consider potential requests from homeless persons within both the "at risk" as well as the "homeless" Categories 2, 3 and 4. Note that persons within these homeless categories are not eligible for Rapid Re-Housing assistance.
- Application V, Rapid Re-Housing DOWNLOAD (1 PDF Document, as needed) - Do not scan this document. Use your agency's HMIS Program Key and name document as follows: PROGRAM_KEY_ReHousing.pdf.
- Note that there is no funding limit for the Rapid Re-Housing program. DCA expects applicants to develop a reasonable business plan for their program based on match resources, the projected number of households to be served, costs per household and the cost of service personnel necessary to effectively carry out the program. In making awards, DCA reserves the right to scale down proposals.
- Applicants are cautioned that only Category 1 homeless persons may be served with Rapid Re-Housing Funds! In program design, applicants may wish to look at Homelessness Prevention if they wish to serve homeless persons within Categories 2, 3 and 4.

- Application VII, Street Outreach DOWNLOAD (1 PDF Document, as needed) - Do not scan this document. Use your agency's HMIS Program Key and name document as follows: PROGRAM_KEY_Outreach.pdf
Naming Your PDF and ZIP files:
AGENCY_KEY and PROGRAM_KEY are the Unique Identifier Numbers used in PATHWAYS and should match the Agency Key and Program Keys listed in your 2012 Application Summary.
If your agency or program is new, not yet participating in HMIS or if it is a program associated with a family violence agency, you should use your Organization Name and Program Name to name your forms and zip file. For example (fictional, of course), "Cobb Family Violence, Inc." would use the following naming convention in applying for funds to assist with the operation of its program called "Marietta House:" Cobb_Family_Violence_Marietta_House_Shelter.pdf.
Once all of your required Forms have been Saved, you need to create a Compressed Zip Folder that contains all of your Application Documents and Upload to HTFOnline using the same URL, Login and Password used for Organizational Documents.
Example for document naming where Agency Key is 9999 and the transitional housing Program Key is 0123:

Example for document naming where agency and program are not in Pathways:

Note that if your agency is also a HOPWA applicant, you must include the HOPWA Application, appropriate local approvals and consolidated plan certification(s), and the HOPWA narrative within this same zip folder.
