Alternative Dispute Resolution and Annexation Arbitration Forms

Use these interactive forms for all work relating to the ADR processes required for the resolution annexation disputes and other interjurisdictional conflicts.  DCA will not accept forms that deviate in format from the files on this site. If you need additional space to answer the questions, please attach a separate sheet with the corresponding page and question number.
